Being a specialist is considered a tradition to emperors/empresses, and it serves a practical purpose as a way for the emperor/empress to defend themselves without assistance, just in case they are separated from their guards. Ghara decided to be a medical specialist as he would alway be prepared for any situation where a doctor or at least a medic was needed.

I actually don't know what particular training Cetiri had from her time in the Survey Corps... probably survival training, but I'm not sure what else... hn...

As stated above, the Imperial Throne can be inherited by any member of the Aristocracy Families, through the process of Pulling A Name Out Of A Hat But Fancy. It's why this is the 8th Hasran Dynasty, as the end of each dynasty is determined by each monarch of the dynasty. When a monarch decides to end a dynasty upon their death/retirement, they just pull a name out of a carefully maintained, ancient helmet.
